Movement and Rehabilitation for Dogs and Cats: Physical Therapy That Supports Joint Health

Over the last two weeks we've looked at how to recognize pain in a dog or cat, and at the herbs, omega-3s, and gut support that may help ease a body carrying extra inflammation. This week closes the series with the piece that gets treated as an afterthought more often than it should: movement itself. Done thoughtfully, exercise and rehabilitation are not something you add once the pain is under control. They are part of how a joint stays functional in the first place.

Movement and rehabilitation for dogs and cats covers more ground than most pet parents expect: structured exercise programs, hydrotherapy, pulsed electromagnetic field (PEMF) therapy, and a handful of simple things you can do at home between veterinary visits. None of it replaces a diagnosis or a treatment plan from your vet. Most of it works alongside one, and some of it is the kind of daily habit that costs nothing but a few minutes and a little consistency.

Why Movement Belongs in the Plan, Not Just Products

It is easy to think of joint support as something that comes in a bottle: an omega-3, a curcumin capsule, a joint chew. Our earlier piece on natural anti-inflammatory support for dogs and cats covered exactly that ground. But a body that is not moving well, even a body that is well supplemented, tends to lose muscle around a sore joint, and less muscle means the joint itself absorbs more of the load with every step.

In my practice, this is one of the more common gaps I see. Families do the diet work. They add the supplements. Then the dog still gets a slow, cautious ten-minute walk around the block once a day, because that feels safer than doing too much. Often, the opposite problem is quietly forming: a joint and the muscle around it that are losing ground from too little structured use, not too much.

Structured Exercise: What the Research Actually Shows

A 2026 prospective clinical study published in Frontiers in Veterinary Science followed 21 dogs with degenerative joint disease through a five-week structured therapeutic exercise program, measuring joint range of motion and limb muscle circumference before and after. Hip extension showed the most consistent improvement, with gains recorded in roughly two-thirds of the hip joints evaluated, and the thigh muscles around the pelvic limbs showed the largest circumference increase of any area measured.[1]

The researchers were careful to note real variability between individual dogs, which lines up with what I see clinically. Two dogs with a similar diagnosis rarely respond identically to the same program. That is part of why a generic online routine is less useful than one built around your own pet's joints and current fitness level.

A broader 2022 review in Veterinary Sciences looked at the overall evidence behind physiotherapeutic strategies for canine osteoarthritis and reached an honest, somewhat humbling conclusion: canine-specific research still lags well behind what exists for people, even though the biological logic, more muscle strength and better proprioception around a joint, carries over reasonably well. The reviewers pointed out that active exercise is generally preferred over passive techniques, and that regular, appropriately paced walking remains one of the more evidence-backed pieces of a joint plan.[2]

Good to Know

Consistency tends to matter more than intensity. A dog who gets a short, structured session most days of the week generally does better than one who gets a long, ambitious hike once on the weekend.

Hydrotherapy: Low-Impact Movement in the Water

Hydrotherapy, most often delivered through an underwater treadmill at a rehabilitation facility, lets a dog move through a fuller range of motion with much less weight actually pressing down on the joint. For an overweight or acutely painful dog, that buoyancy can make a session possible that would be too much on dry land.

A 2025 pilot study published in the journal Animals reviewed records from 50 dogs who completed a 10-session underwater treadmill program over five weeks. Passive range of motion, measured with a goniometer, improved significantly across every joint evaluated, from the carpus to the hip, and the improvement held regardless of the dog's age or underlying diagnosis, whether that was hip dysplasia, spondylosis, or disc disease.[3]

The same 2022 Veterinary Sciences review mentioned above is more cautious here, noting that while hydrotherapy clearly offers low-impact, higher-intensity movement, the long-term evidence on joint stiffness and comfort specifically is still thin.[2] I read that as a reason to treat hydrotherapy as a genuinely useful tool, not a guaranteed fix, and to work with a facility that tailors water depth and session length to your individual pet rather than running every dog through the same protocol.

Pulsed Electromagnetic Field (PEMF) Therapy

PEMF therapy uses low-level electromagnetic pulses, delivered through a mat or loop, with the aim of supporting cellular activity in a way that may ease discomfort. It has become a regular part of how I support pets with chronic joint pain, and I currently use Pets PEMF devices in my own practice.

A 2025 randomized clinical study in Veterinary Research Communications assigned 20 dogs with hip osteoarthritis to either active PEMF treatment or a control group, twice weekly for five weeks. Pain scores on two separate validated indices improved meaningfully in the treated dogs, starting as early as the first few sessions. It is worth being precise about what the study did not find: there was no significant difference in joint range of motion or thigh muscle circumference between groups.[4] In other words, the pain relief looked real in this trial. The evidence for PEMF rebuilding joint function on its own did not.

Worth Noting

I use PEMF as one layer of a broader plan, alongside structured exercise, weight management, and whole-food nutrition, not as a stand-alone answer. A tool that eases pain still needs muscle-building and joint-loading work happening around it.

Weight Management as a Movement Multiplier

Of everything covered in this article, weight management has some of the oldest and most consistent evidence behind it. A landmark study published in Veterinary Research Communications followed 14 obese dogs with hip or elbow osteoarthritis through a 16-week weight reduction program. Lameness scores began improving once dogs had lost roughly 6 percent of their starting body weight, and by the end of the study, 82 percent of the dogs showed measurable improvement on standardized lameness scoring.[5]

This is why weight and movement belong in the same conversation instead of two separate ones. Every extra pound a dog carries is extra load traveling through an already compromised joint with every single step. A TCVM Lens: Wood, the Liver, and the Joints

In Traditional Chinese Veterinary Medicine, the Wood element governs the liver and is closely tied to the joints, tendons, and the smooth, unimpeded flow of movement through the body. A pet with a strong Wood imbalance often shows up as irritable or frustrated, with joint stiffness that seems to ease once the body actually gets moving, and sometimes an irregular bowel pattern alongside it.

I do not expect every pet parent to learn Chinese medicine theory. I do ask them to notice whether their dog seems stiffest first thing in the morning and looser after ten minutes of walking. That single pattern, stiff at rest and better with steady movement, is a classic Wood presentation, and it is also exactly the profile that tends to respond well to a consistent, gentle daily movement routine rather than sporadic bursts of activity.

Simple Home Exercises to Start This Week

None of what follows should replace a professional's guidance for a pet with a diagnosed orthopedic or neurologic condition. For a dog or cat who is otherwise stable and cleared for activity by their vet, these are reasonable starting points.

  • Slow, controlled leash walks on level ground, five to ten minutes, two to three times a day, rather than one long walk that leaves your pet sore afterward.
  • Sit-to-stand repetitions, five to eight at a time, once or twice a day, which builds hindquarter strength without any equipment.
  • Gentle figure-eight walking around two chairs or cones, which asks the hips and shoulders to work through a slightly wider range of motion than a straight line does.
  • Wobble cushion or folded towel standing, thirty to sixty seconds at a time, for a dog who tolerates it well, to build core and joint stability.
  • For cats, a wand toy used low to the ground a few minutes a day encourages stretching and weight shifting through the hips and shoulders without the impact of a jump.
  • A short daily mobility check, watching how your pet rises from lying down, noting any new hesitation or favoring of one leg.
Worth Noting

More is not automatically better. If your pet seems more sore the day after a session, that is useful information, not a sign to push harder. Scale back and give the body a rest day before trying again.

When to Bring in a Certified Rehabilitation Professional

Home exercises are a reasonable starting point for a stable pet, but they are not a substitute for hands-on guidance once a condition is more involved, a pet is recovering from surgery, or progress has stalled. The American Association of Rehabilitation Veterinarians maintains standards for the field and points pet parents toward credentialed practitioners, including Certified Canine Rehabilitation Therapists and Certified Canine Rehabilitation Veterinary Nurses, trained through supervised, hands-on coursework in therapeutic exercise, manual therapy, and modalities like hydrotherapy and PEMF.[6]

If your dog or cat is not making the progress you expected after a few weeks of home exercise, or if you are not confident you are doing an exercise correctly, that is the right moment to ask your veterinarian for a referral to a certified rehabilitation practitioner rather than continuing to guess.

How These Pieces Come Together in Practice

A pattern I see often, more than any single case, is a dog managing hip or elbow osteoarthritis who is comfortable enough at rest, already on a prescribed medication or joint supplement, but who has grown hesitant on stairs. Just as often, the family has quietly scaled back walks out of caution, not because a vet told them to, but because slowing down felt like the safer choice.

Working alongside a pet's regular veterinarian, a plan for a dog like that usually layers a few things rather than reaching for one fix: keeping any prescribed medication in place, building a short-walk routine back up gradually, adding simple strength work like sit-to-stand a few mornings a week, and, where a vet supports it, a modality like PEMF for pain. Addressing any extra weight that crept in during a less active stretch is often part of the same conversation, given what the research in Section 5 shows about weight and lameness.

Families who stick with a plan like this consistently over several weeks often describe a dog who takes the stairs more readily and asks for a walk again. That said, every dog's diagnosis, age, and starting point are different, and this is a general pattern rather than a guarantee for any individual pet. Your veterinarian, or a certified rehabilitation professional, is the right person to say what pace and combination make sense for your own dog or cat.
